Tesofensine is an oral small molecule that blocks the reuptake of three neurotransmitters — serotonin, noradrenaline, and dopamine — which suppresses appetite and increases energy expenditure. It was first investigated for Parkinson’s and Alzheimer’s disease, where researchers noticed marked weight loss as a side effect.
In the Phase 2 TIPO-1 trial, tesofensine produced roughly 10% mean weight loss at 24 weeks — about double what other agents achieved at the time. It is being developed further (including for rare hypothalamic obesity) but is not FDA approved for general weight management.
