What Is Ozempic?

Ozempic is the brand name for semaglutide — a glucagon-like peptide-1 (GLP-1) receptor agonist administered as a once-weekly subcutaneous injection. Developed by Novo Nordisk, it was FDA-approved in December 2017 for glycaemic control in adults with type 2 diabetes. The same molecule at higher doses is marketed as Wegovy for chronic weight management.

GLP-1 is an incretin hormone naturally released by intestinal cells after eating. Semaglutide mimics GLP-1 but has a half-life of approximately 7 days (vs. minutes for native GLP-1) due to its fatty acid chain and albumin binding, making once-weekly dosing effective. It reduces appetite via hypothalamic receptors, slows gastric emptying, and stimulates glucose-dependent insulin secretion.

Ozempic vs Wegovy vs Rybelsus

All three are semaglutide. Ozempic (0.5–2 mg injectable) is approved for type 2 diabetes. Wegovy (2.4 mg injectable) is approved for obesity. Rybelsus (3–14 mg oral) is approved for type 2 diabetes. Track whichever formulation your prescriber uses in Shotlee.

Ozempic Dose Escalation Schedule

0.25 mg

Weeks 1–4

Starter dose — not therapeutic. Allows GI tolerance to develop before escalation.

0.5 mg

Weeks 5–8+

First maintenance dose. Many patients achieve target HbA1c reduction at this level.

1 mg

Week 9+ if needed

How to Inject Ozempic

Choose a site: abdomen (at least 2 inches from navel), thigh, or upper arm. Rotate sites weekly.

Clean the skin with an alcohol swab and allow to dry for 10–15 seconds.

Remove the cap, prime the pen if it is a new one (until a drop appears at the needle tip).

Pinch the skin, insert at 90°, press and hold for 6 seconds, then withdraw. Apply gentle pressure — do not rub.

Common Side Effects & Management

Nausea

Most common (17–20%). Usually worst in the first 4–8 weeks. Eat smaller meals, avoid high-fat foods, stay hydrated. Subsides in most patients by week 12.

Vomiting

Occurs in ~5–9% of patients. If persistent, discuss with prescriber — a dose delay or reduction may help more than antiemetics.

Diarrhoea / Constipation

GI motility changes are common. Increased fibre and hydration help. Track stool frequency changes in Shotlee to share patterns with your provider.

Injection Site Reactions

Mild redness or itching at injection sites is normal. Rotate sites every week. Persistent lumps may indicate lipohypertrophy — avoid injecting the same spot.

Headache / Fatigue

Rare: Pancreatitis

Seek emergency care for severe abdominal pain radiating to the back. GLP-1s carry a label warning; absolute risk remains low in clinical populations.

Blood sugar improvements begin within days of the first dose. Meaningful HbA1c reduction is typically seen by 12 weeks. Weight loss effects accumulate over 3–6 months. The 0.25 mg starter dose is sub-therapeutic — effects build after escalation to 0.5 mg and above.

Ozempic is FDA-approved for type 2 diabetes only. Wegovy (semaglutide 2.4 mg) is FDA-approved for chronic weight management. Some prescribers prescribe Ozempic off-label for obesity. For FDA-approved weight loss treatment, ask your prescriber about Wegovy. Track either in Shotlee.

Take the missed dose as soon as you remember, provided the next scheduled dose is at least 2 days away. If the next dose is within 2 days, skip the missed dose and resume the regular schedule. Never take two doses in the same week.

Rapid weight loss from any cause can include lean mass loss. Studies show semaglutide users lose approximately 25–40% of lost weight as lean mass, similar to other methods. High protein intake (≥1.2 g/kg body weight) and resistance training are the primary mitigation strategies. Track both weight and measurements in Shotlee to distinguish fat vs mass changes.

Unopened pens: refrigerate at 2–8°C until the expiry date. In-use pen: store at room temperature (below 30°C) or refrigerate for up to 56 days. Never freeze. Protect from light. A travel case with a cool pack is recommended for trips over 30°C.
